Performance Appraisal
Managerial Level - Guidelines for Completion of the Managerial Level Performance Appraisal Form

  1. Upon receipt, please distribute to direct reports, i.e, assistant vice presidents, assistant deans, executive directors, directors, bursar, registrar, associate directors (where appropriate)
  2. Have direct reports complete parts I, II, III, Va and Vb
  3. Set up an appointment date to discuss and complete parts IV and VI
  4. Complete part VI
  5. Have direct reports complete part VII
  6. Submit entire packet to the appropriate Vice President
  7. Attach a salary recommendation list for your area for discussion with the appropriate Vice President after all appraisals are completed
