Work Related Injury Procedures

In an effort to familiarize you with the procedures governing work-related injuries, we would like to advise you of the following: 

1.      In the event that you are injured in the course of your employment, you are required to notify your immediate supervisor and the Office of Payroll and Benefits, at extension 2124, as soon as possible.

This notification should be done no later than the end of the workday on which the injury occurred. 

2.   A State of NJ accident report (form RM-2) must be completed and submitted to the Office of Payroll and Benefits. In order for you to be considered for Workers’ Compensation benefits, State regulations require that this completed and signed accident report be received in the Office of Payroll and Benefits within 48 hours after the injury occurred. Maintenance and University Police staff may obtain the form in their respective department; all other employees may obtain the form from the Office of Payroll and Benefits. 

  1. For treatment of a work-related injury or illness that is not a medical emergency, it is the policy of the University to refer employees to the Immedicenter located at 500 Union Boulevard, Totowa, NJ, which is open Monday to Friday 8 a.m. to 8 p.m. and Saturday 9 a.m. to 3 p.m. Another Immedicenter located at 1355 Broad Street, Clifton, NJ, is open every day of the year from 8 a.m. to 10 p.m.  

In the event of a medical emergency or if medical treatment is required after the operating hours of the Immedicenter facilities, employees should seek treatment at St. Joseph’s Wayne Hospital’s Emergency Room. 

The State of New Jersey has selected Horizon Casualty Services to provide a managed care program to administer the University’s Workers’ Compensation claims.  An injured employee must follow the above reporting procedures and seek treatment in accordance with this managed care program. Employees who do not comply with these procedures will be responsible for any unauthorized medical bills. 

Prior to any Sick Leave Injury (SLI) absence due to a work-related injury being approved, the employee must have followed the above required procedures. It is the employee's responsibility to provide to the Office of Payroll and Benefits the proper medical documentation from an approved physician supporting the absence. 

In cases where there is reason to believe that work-related sick leave is being abused, the University or its designee may investigate. If the University concludes that there has been an abuse, disciplinary action, up to and including termination, will be taken. In addition, where appropriate, the matter will be referred to the State of New Jersey’s Attorney General’s Office for legal action. 

I have read and understand the above work-related injury reporting procedures and agree to comply with all of the procedures listed.
