I have received a copy of the William Paterson University Child Development Center Family Handbook.

The Director explained the role and responsibilities of families. Family participation in many aspects of our program is an important component.

I have received a registration package as required by the State of New Jersey Bureau of Licensing, Division of Youth and Family Services that includes:

• Registration form
• Emergency Medical Release Form
•Immunization/Health Record
•Permission Slip
•General Information Form
•Parking Permit
•Tuition Policies

I have received a fifteen-minute parking permit for the sole purpose of picking up/dropping off my child.

I understand that I have the right to discuss any concerns or recommendations with the Director.
