Verification Page for Dependent Students

Please submit the following items as soon as possible. Once you submit your documents, please monitor WP Connect for any additional requests and/or your financial aid awards.

1. 2008-2009 Verification Form - Dependent
It is important that this worksheet is fully, accurately and carefully completed. It is a legally binding document and cannot be changed once submitted.

2. Your parents' 2007 Federal Income Taxes (ie: 1040s)
- Please have them SIGNED
- Include all pages and schedules, including business/corporate returns and Form W-2s
- DO NOT include State Returns at this time

3. Your 2007 Federal Income Taxes, if any
- If not, make sure to note it on Verification Form
