My clinical intern is in a co-teaching or multiple grade setting and is not switching placements, what forms do I fill out and how?

If the clinical intern is in a co-taught (SPED and Gen Ed or Bilingual and Gen Ed team teaching) or is in a K-8 school for Phys Ed or Art (doing both the elementary and secondary in the same school) then you could follow the following:

After Eight Weeks (halfway point)
4 Green Observation Forms (2 Gen Ed/2 SPED or Bilingual) (2 elementary/2 secondary)
2 Pink, Rated Interim Reports (1 Gen Ed/1 SPED or Bilingual) (1 elementary/1 secondary)
1 SPA Form

After Sixteen Weeks (end)
2 Green Observation Forms (2 Gen Ed/2 SPED or Bilingual) (2 elementary/2 secondary)

My clinical experience student is completing two clinical experiences in one semester.  What forms do I fill out?

Each clinical experience needs to be treated separately.  Complete the two observation forms, one rated final report and one SPA form for each clinical experience.

 

Why do I have extra SPA forms?

You are given two sets of SPA forms.  Please provide one SPA form to the clinical educator for clinical experience students and two SPA forms for clinical practice teachers.  The clinical educators are to fill out the SPA forms at the points in the semester as you.

 

My clinical intern is struggling in their placement, what do I do?

Contact the Office of Field Experiences immediately if you have a student is having difficulty and/or the clinical educator expresses concern.  Make sure to document any concerns you may have on the observation form as well as in e-mails.  Please fill out the Special Case in Field Experiences form and submit to the Office of Field Experiences immediately.
